The 2026 Part D $2,100 Drug Cost Cap Explained for Fayetteville, NC Medicare Beneficiaries

The limit covers qualifying Part D drug costs, not your entire Medicare budget.

The bottom line

  • The threshold is $2,100 in 2026, up from $2,000 in 2025.
  • After you reach it, you pay no cost-sharing for covered Part D drugs for the rest of the calendar year.
  • Premiums, non-covered drugs and medical expenses are separate from this drug-cost limit.
  • Compare the full annual estimate for your prescriptions and pharmacies, not just a premium or tier label.

What the $2,100 cap covers

The limit applies to covered Part D prescriptions, whether you have a stand-alone drug plan or a Medicare Advantage plan with drug coverage. Not every Medicare Advantage plan includes Part D. Check your coverage before comparing costs.

Qualifying deductible, copay and coinsurance payments count toward the threshold. Certain payments made on your behalf also count. Your monthly premium does not. Check your plan's Explanation of Benefits to see what has accumulated toward the limit. See Medicare's drug coverage cost guide.

The changes took effect in stages. Catastrophic-phase beneficiary cost-sharing was eliminated in 2024. The annual threshold was $2,000 in 2025 and increased to $2,100 in 2026. See the CMS 2025 announcement and CMS final 2026 Part D instructions.

The cap limits the year's qualifying drug costs; it does not mean every prescription fill is affordable early in the year. The Medicare Prescription Payment Plan can spread covered out-of-pocket costs across the calendar year, but does not reduce their total.

Compare your prescriptions, not just tier labels

For a Fayetteville or Cumberland County plan review, prepare a list of drug names, strengths, quantities and refill schedules, plus the pharmacies you use. A formulary is a plan's covered-drug list. Its tier labels are a starting point, not a complete price comparison.

  1. Choose the coverage year. Use your ZIP code in Medicare Plan Compare. When shopping for next year's coverage, select that year rather than assuming the 2026 limit still applies.
  2. Enter each prescription. Check the exact drug and dose, and confirm prior authorization, quantity limits and other requirements in the plan documents.
  3. Compare pharmacies. Ask for estimates at your usual pharmacy and any alternatives you would realistically use.
  4. Compare the annual total. Include premiums, deductibles and expected prescription cost-sharing. Ask separately about costs for drugs the plan does not cover.
  5. Confirm before enrolling. Save the estimate and ask the plan to confirm coverage details. A lower tier or deductible alone does not guarantee a lower annual bill.
Key takeaway: The shared annual cap does not make plan premiums, pharmacy prices or prescription coverage interchangeable.

IRMAA is a separate premium cost

An Income-Related Monthly Adjustment Amount (IRMAA) may increase your Part D premium based on income. Social Security generally uses tax information from two years before the premium year, as explained in the SSA IRMAA handbook. This premium amount does not count toward the covered-drug cap.

A qualifying life-changing event that reduces income may support a request for a lower amount. Follow Social Security's review instructions; a large income event does not automatically impose exactly two years of surcharges.

Frequently asked questions

What is the 2026 Part D $2,100 out-of-pocket cap?

The annual out-of-pocket threshold for covered Part D drugs increased to $2,100 in 2026, from $2,000 in 2025. After reaching it, you pay no cost-sharing for covered Part D drugs for the rest of the calendar year. Premiums and non-covered drugs are separate expenses.

Does the cap apply to Medicare Advantage drug coverage?

Yes. The Part D limit applies to Medicare Advantage plans with Part D drug coverage as well as stand-alone Part D plans. It is a drug-cost limit, not a limit on medical costs or premiums; confirm that your particular plan includes Part D.

Is a lower formulary tier always the less expensive choice?

No. Compare the estimated annual cost of your exact prescriptions together with premiums, deductibles, pharmacy pricing, and coverage requirements. A tier label alone does not establish which plan costs less for you.

Does Part D IRMAA count toward the drug-cost cap?

No. The income-related monthly adjustment amount (IRMAA) is an additional premium amount, not covered-drug cost-sharing. Social Security generally uses tax information from two years before the premium year; contact Social Security about a review if a qualifying life-changing event reduced your income.
